You can now print the plain paper versions in addition to printing on AIA Oiginal versions of these same forms. This multi-page .pdf contains a completed sample of each of the forms mentioned above. These forms should be used in conjunction with the instructions on how to complete a Certified Payroll Report, found at http://www.sunburstsoftwaresolutions.com/certified-payroll-reporting.htm This Union Specific Fringe Benefit Report was designed and completed using Certified Payroll Solution's "Benefit Reporting" module. The Washington State Department of Transportation has a Monthly Employment Utilization Report (DOT Form 820-010 EF) which has been added to Certified Payroll Solution January 2010. The Colorado Division of Small Business Opportunity (DSBO) has released a Monthly Compliance (Workforce) Activity Report (COMP-FRM-24). This report was added to Certified Payroll Solution January 2010.

The Statement of Compliance for the Missouri Department of Labor and Industrial Relations Division of Labor Standards Form LS-57 Contractor Payroll Records is quite complex, as it requires contractors to report the hourly rates of each type of fringe benefit that is either paid to the Union Hall or a bona-fide plan on behalf of the employee shown on the certified payroll report. Additional requirements include the reporting of the name of the Plan, Fund, or Programs to which these fringe benefits are paid.
Effective January 1, 2010 the Missouri Department of Labor and Industrial Relations Division of Labor Standards has developed and is now requiring that contractors working on State funded construction projects complete Form LS-57 Contractor Payroll Records. Previously Missouri followed the reporting requirements of the U.S. Department of Labor for all state funded construction projects with the exception of those funded by the City of Kansas City. This is a Legal-sized form.
